Our patients have challenging health conditions that require more than basic primary care. AFC patients have access to our wide network of medical specialties — everything from cardiology and pulmonology to oncology and surgery — provided pro bono by generous medical professionals in the community. Also integrated into our comprehensive medical care model are: mental health services, physical therapy, pharmaceuticals, education (nutrition, exercise, diabetes) and oral health care.
Many patients are also dealing with one or more of the following: language/literacy barriers; food, housing or employment insecurity; or anxiety and depression. Our clinical staff coordinates every aspect of the care our patients require, and they do so within the context of patients’ social and cultural needs.
