Holistic Programs Addressing Social Determinants of Health (SDOH)

We also understand that healthcare is only one aspect of being healthy. Our patients often face challenges related to housing, food insecurity, and employment, all of which impact their health. In response, we provide a variety of support services, including:

Case Management and Social Services

Our case management team works closely with patients to connect them with resources outside of healthcare, including:

  • Housing assistance
  • Food programs
  • Employment support
  • Legal aid

This holistic approach ensures that patients have the support they need to maintain stability in their lives and focus on their health.

Health Education and Wellness Programs

AFC places a strong emphasis on health education and wellness programs as part of our mission to empower patients. Through workshops, group classes, and one-on-one counseling, we provide education on topics such as:

  • Smoking cessation
  • Chronic disease management

Many of our education materials are available in both English and Spanish to ensure accessibility for all patients. These programs are designed to give patients the tools they need to take control of their health, improve self-care, and prevent future complications.
