Arlington Free Clinic was awarded a three-year $350,000 grant from CareFirst BlueCross BlueShield (CareFirst) to support AFC’s transition to a Patient Centered Medical Home (PCMH) model of care. The grant was celebrated with a check presentation from CareFirst held at Arlington Free Clinic.
The primary goals of Arlington Free Clinic’s PCMH are to produce measurable improvements in patient care delivery and population health outcomes, and to optimize the use of on-site and community-based resources that are donated by community partners. AFC’s grant is part of CareFirst’s Safety Net Health Center Patient-Centered Medical Home Initiative, announced in February of this year, that awarded more than $8.5 million in funding over three years to help 12 safety net health center programs in Maryland, Virginia and Washington, D.C. AFC was the only clinic in Virginia to receive support for the Patient Centered Medical Home initiative.
